Resumen

A pesar de que la información disponible en la literatura sobre la embarazada con epilepsia ha proliferado en los últimos años, aún hay mucho por hacer.

El propósito de este artículo es revisar la literatura y describir consideraciones prácticas sobre el enfoque y manejo de la embarazada con epilepsia.

El tratamiento de la mujer embarazada con epilepsia constituye un importante reto clínico. El tratamiento por un equipo interdisciplinario entre obstetras, ginecólogos, neurólogos y en su momento, neonatólogos, es una condición fundamental para disminuir el riesgo que presentan las pacientes, pues muchos problemas pueden prevenirse.

El embarazo debe ser planeado en lo posible para recibir la consejería pertinente. Esto con el fin de formular suplementos de ácido fólico, mantener la adherencia al tratamiento anticonvulsivante, aclarar los riesgos de teratogenicidad así como la importancia del cuidado preconcepcional, antepartum y postpartum. El ajuste de dosis de anticonvulsivante debe basarse en aspectos clínicos y no en niveles séricos.

Muchos grupos han sugerido guías de manejo clínico para estas pacientes. A pesar de que el futuro es prometedor en esta área, aún persisten algunas preguntas y el interés por progresar en este campo
